Pre-Event Medical Screening Checklist.

Completion of this form is required for all individuals who will be attending or participating in a Troop 2016 event (even when conducted in conjunction with a public activity). The parent or guardian must complete the form on behalf of minors.

This form should be submitted no sooner than 24 hours before the event.

If you have any communicable medical conditions, please follow the CDC guidelines for isolation (if necessary) and your personal healthcare provider's treatment recommendations.
